Individuals with nephroptosis suffering from nephrotic symptoms can have pain in the upright position which worsens with movement and tends to get more severe the longer the individual is upright. Pain severity can tend to increase over time in some individuals if nephroptosis is left untreated.

When a person with nephroptosis experiences violent episodes of renal colic with symptoms of blood and protein in the urine, nausea, vomiting, rapid heart beat (tachycardia), and chills this is known as Dietl's crisis. These symptoms can be caused by an obstruction or kink in the ureter.
